To order an Elvarex Custom Toe piece please download the order form, complete, save and return to info@apemedical.com.au or fax to (08) 9467 2868.
Please be very careful with your measuring and ensure all measurements are filled in correctly. There are no alterations of finished garments and no refunds provided for incorrectly measured garments.
We will send through a complete quote for your approval. Once approved we will place the order directly with JOBST however payment must be made prior to manufacturing. Payment information will be on the quote sent to you.
We are able to bill the NDIS directly for all approved quotes. Read more information on the NDIS process.
