Smart Electrodes
Form comes with two bespoke electrodes which have been designed to provide optimum muscle coverage. The small and large electrodes allow for use on different sized muscles.
The ‘smart electrodes automatically configure the programme parameters depending on the selected muscle group.
With correct application, the electrodes maximise motor point activation to elicit small muscle contractions. This results in effective muscle recovery in just 20 minutes.
Our innovative electrodes use reusable and replaceable hydrogels which provide between 10-20 applications before needing to be replaced, minimising waste and maximising effectivity.

FAQ
Q: Why won’t the display turn on?
A: The battery may be flat, try charging your Form. If this does not work, contact us.
Q: Why won’t the stimulator connect to the electrode?
A: The stimulator is in the wrong orientation. Rotate the stimulator so the shapes on the bottom of the stimulator and top of the electrode align. This should allow the magnets in the stimulator and electrode to attract. See section 4 of your instruction manual.
Q: Why won’t the stimulator connect to the charging cradle? The stimulator is in the wrong orientation. Rotate the stimulator so the shapes on the bottom of the stimulator and top of the electrode align. This should allow the magnets in the stimulator and charging cradle to attract.
Q: Why won’t the electrodes stick?
A: Ensuring you prepping the skin correctly and placing electrodes per the electrode guide in the instruction manual. If electrodes do not stick in correct placement your hydrogel pads may have lost their adhesion and will need replacing.
Q: Why does the sensation feel low / inconsistent?
A: Your hydrogel pads may have lost their adhesion and will need replacing. Stimulation cannot pass properly through expired electrodes.
Q: Why is there no sensation?
A: Ensure your Form is charged. The intensity may not be strong enough, try increasing the intensity. There could be an incorrect connection between electrode, stimulator and body, ensure the electrode is effectively attached to the body. Ensure connection between stimulator and electrode. If electrodes will not easily adhere to the body for a full session you may be using old hydrogels pads and need to replace them.
Q: Why is the display flashing?
A: This can be due to three reasons; Low battery – ensure your Form is charged. Connection error between stimulator and electrode – disconnect and reconnect the stimulator and increase the intensity. Connection error between electrode and body – ensure the electrodes are properly applied, replace hydrogel pads if necessary.
Q: Why can’t I change the intensity / why is the rotating dial not working?
A: The stimulator is locked. Unlock the stimulator by double-pressing the control button.

Contradictions
- Do not use if you have a heart pacemaker, (or if you have a heart rhythm problem) or with any electronic medical devices. Using this unit with electronic medical devices may cause erroneous operation of the device. Electrical stimulation in the direct vicinity of an implanted device may affect some models.
- Do not use during the first three months of pregnancy. It is not known whether EMS may affect foetal development.
- Do not use on the abdomen in the later stages of pregnancy. Stop using immediately if you experience unexpected contractions.
